IRRITABLE BOWEL SYNDROME

Irritable bowel syndrome (IBS) is a functional bowel disorder in which abdominal pain is associated with defection or a change in bowel habit. Approximately 20% of the general population fulfills diagnostic criteria for IBS but only 10% of these consult their doctors because of gastrointestinal symptoms. Nevertheless, IBS is the most common cause of gastrointestinal referral and accounts for frequents absenteeism from work and impaired quality of life. Young women are affected 2-3 times more often than men. Co-existing conditions such as non-ulcer dyspepsia, chronic fatigue syndrome, dysmenorrhoea and fibromyalgia are common.

Diagnosis

The diagnosis is clinical in nature and can be made confidently in most patients
under the age of 40 years without resorting to complicated tests. 
Colonoscopy should be undertaken in older patients to exclude colorectal cancer.
Features of irritable bowel syndrome                                          

Alarm features

Colicky abdominal pain

Altered bowel habit

Feeling of incomplete defection

Abdominal distension

Rectal mucus

 

 

                  Supporting diagnostic features and alarm features in IBS

Features supporting a diagnosis of IBS:

·        

Symptoms > 6 months 

·        

Previous medically unexplained symptoms

·        

Frequent consultations for non-G problems  

·        

Stress worsens symptoms

 

Alarm features

·        

Age > 50 years; male gender

·        

Family history of colon cancer

·        

Weight loss

·        

Anemia

·        

Nocturnal symptoms

·        

Rectal bleeding

Management

The most important steps are to make a positive diagnosis and reassure the patient. Many patients are concerned that they have developed cancer, and a cycle of anxiety leading to colonic symptoms, which further heighten anxiety, can be broken by explanation that symptoms are not due to organic disease but are the result of altered bowel motility and sensation. In patients who fail to respond to reassurance, treatment is tailored to the pre-dominant symptoms.

Drug therapy for IBS

Evidence for efficacy of drug therapy for IBS is weak. There is evidence of benefit for antispasmodic drug for abdominal pain and overall response but there is no clear evidence of benefit for antidepressants or bulking agents. For each individual, treatment should be aimed at the most debilitating symptoms.

Laylatul Baraat - Shab e Baraat 15th Shaban  1433 today

The Holy Shab-e-Baraat, the night of reverence and fervour and divine blessings for the Muslims, would be observed on Thursday night (today). The Shab-e-Baraat is the night of blessings and worship of the Allah, the 15th night of Shaban, eighth month of the Islamic lunar calendar, is known as Shab-e-Baraat while it is known as Laylatul Bara'ah or Laylatun Nisfe min Sha'ban in the Arab world. 

Muslims typically celebrate Shab-e-Baraat on the 15th of Shaban, usually around 15 days before the month of Ramazan begins. Shab-e-Barat has tremendous benefits for Muslims. It is best to pray and worship Allah on that night and seek his forgiveness. Muslims pass the night (Shab-e-Baraat) offering special prayers, recitation from the Holy Quran, holding Milad Mahfil, Zikr, and other religious rituals to mark the occasion with due solemnity. 

People also visit the graveyards of their relatives and offer Fateha for the departed souls of their beloved ones. According to a Hadith, Prophet Hazrat Muhammad (PBUH) said that any individual who fasts for three days during the month of Shabaan and recites Durood three times at Iftar, Allah will forgive his sins and will increase his provisions. Additionally, the person will enter Paradise on a female camel on the Day of Judgement.

4 Myths about Your Ideal Weight

Have you put on your bathing suit yet--or shorts, or tank top, or other summer clothing?  If you're feeling down about how you look, then this blog is for you! I recently read this list of myths from my fellow editors at Best Health magazine. Don’t we all fall for these at one point or another?

Myth 1: My ideal weight was when I graduated college, or before I had kids.

If you’re hoping to get back to what you weighed a few years ago, fine. But if you’re looking at 10 or more years down memory lane, stop. Many people put on weight as they get older, and a slower metabolism makes it all the harder to slim down as easily or as quickly as you did in the past. Don't live in the past! Set a goal that works for the way you live now.

Myth 2: I'll find my ideal weight on a standard height and weight chart.

Many factors play a role in determining your weight, such as your body type, the number of fat cells you have, how muscular you are, et cetera. The numbers on a standard body mass index (BMI) chart are just approximations, and may not be the best gauge of good health. Studies show they may undercount some women as overweight by not measuring body fat and over count others who have a higher ratio of muscle to fat.

Myth 3: My ideal weight is the lowest number I’ve hit on past diets.

The fact that you’re dieting again means that you’ve gained some, if not all, of the weight back. If you set a weight-loss goal that’s too low to maintain, you’ll get caught in an unhealthy vicious cycle of yo-yo dieting. Such repeated weight loss and regaining can alter your body composition, lowering the amount of muscle mass you have. This, in turn, can slow your metabolism and lower your body’s ability to regulate blood sugar. So what's your best weight goal? The one you can actually live with.

Myth 4: The less I weigh, the healthier I’ll be.

Not true. In fact, many studies show that if you’re overweight, losing just 5 to 10 percent of your current weight is all you have to do to reap the bulk of the health benefits associated with weight loss: lower risk of heart disease, stroke, diabetes, and even some forms of cancer.
